Sounds like it could be a strep infection if you are acutely uncomfortable and running fever, Anytime you are short of breath that warrants a Dr. visit.

It sounds like it could be tonsillitis. This is an infection/inflammation of your tonsils. It usually causes severe sore throat, bad breath, difficulty swallowing, among other things. The white spots could be the exudate commonly found in tonsillitis. However, shortness of breath is not found in tonsillitis and is very dangerous. So, I recommend that you see an M.D. asap.
