Nemesis
To prevent them, gently brush your toothbrush around the area whenever you brush your teeth. It may make you gag, but it should prevent the gunk from building up. Also, brush your tongue, as far back as you can. Follow up with a long gargle of Listerine or salt water.

To get rid of them, the only way is to remove them with either your fingernail, Q-tip, or pair of tweezers. You have to be really careful, because not only could you injure yourself by sticking a sharp object in your mouth, but pulling the stone too quickly could cause damage to the tonsil and the delicate lining inside your mouth. I would only trust myself to use my own fingernail, at least that way I would not have to worry about my hand falling down my throat and choking to death!

If you can't get them out by yourself, you'll have to pay a visit to a dentist.
